
Omega 3 and Brain Health: What DHA Does and Who Benefits
Omega 3 fats, and DHA in particular, are structural building blocks of brain cell membranes, which is why low intake matters. What the evidence does not support is the popular next step: in healthy, well fed adults, omega 3 supplements have repeatedly failed to improve memory or focus in randomised trials. The people most likely to benefit are those starting from a low intake, which in the UK is most of us.

What do omega 3 fats actually do in the brain?
DHA is a physical component of the membrane around every brain cell, where it keeps the membrane flexible enough for receptors and signalling proteins to work properly. It makes up around 90 percent of the brain's omega 3 content and is especially concentrated in the grey matter and the retina, which is why eye and brain requirements travel together.
EPA, the other long chain marine omega 3, does something different. It has little structural presence in the brain but influences inflammatory signalling, which is the route by which most of the mood related research has gone. Treating EPA and DHA as one substance, as most marketing does, blurs two genuinely different jobs.
The requirement is highest when the brain is being built. DHA accumulates rapidly in the last trimester of pregnancy and through the first two years of life, which is why the dietary advice for pregnancy is more specific than it is for anyone else.
Do omega 3 supplements improve memory and focus in healthy adults?
Largely no. Randomised controlled trials in cognitively healthy adults and older people have consistently failed to show a meaningful improvement in memory, attention or processing speed from omega 3 supplements, and Cochrane reviews of this question have concluded there is no convincing benefit for preventing cognitive decline or dementia.
That result surprises people, because the biological story is so tidy. The likely explanation is the gap between deficiency and supplementation. A nutrient that is essential to a structure is not the same as a nutrient that improves that structure when you already have enough, and this pattern repeats across almost the whole of nutrition science.
Where the picture is more interesting is at the low end. Studies of people with genuinely low oily fish intake, and of populations where fish is close to absent from the diet, look more favourable than trials run in well nourished volunteers. If you eat no fish at all, you are in a different starting position from the average trial participant.
Who is most likely to benefit from more omega 3?
The people most likely to benefit are those with a low starting intake, not those chasing an edge on top of an adequate diet. That covers a large share of the UK population, since average oily fish consumption sits well below the recommended one portion a week.
- Anyone who eats no oily fish. If salmon, mackerel, sardines, herring and trout are all absent from your week, your EPA and DHA intake is close to nothing.
- Vegans and most vegetarians. Plant foods supply ALA, not EPA or DHA, and the conversion is poor. Algae oil is the direct fix and it is where fish get their DHA in the first place.
- Pregnancy and breastfeeding. Demand is genuinely higher. UK advice is to eat oily fish, but no more than 2 portions a week during pregnancy, and to avoid shark, swordfish and marlin entirely.
- Older adults with low appetite or a restricted diet. Intake often falls quietly with age, alongside several other nutrients.
Does your omega 3 level change whether B vitamins help the brain?
There is an interesting signal here, and it deserves to be reported with its caveats attached. A secondary analysis of the Oxford VITACOG trial, published in 2016, found that B vitamin supplementation slowed cognitive decline in people with mild cognitive impairment only among those whose baseline omega 3 levels were in the higher range. In those with low omega 3, the B vitamins did little.
What that means honestly: it is a subgroup finding from a trial that was designed to answer a different question, in a specific population, and it has not been confirmed by a trial built to test it directly. It is a good hypothesis. It is not a proven mechanism, and it is certainly not a reason to buy a combined supplement on the strength of the packaging.

How much omega 3 do you need, and where does it come from?
UK advice is at least 2 portions of fish a week, one of which should be oily, which works out at roughly 250 to 500 mg of combined EPA and DHA a day. A single 140 g portion of oily fish typically supplies more than a week's worth on its own, which is why the guidance is framed in meals rather than milligrams.

The conversion problem in one line: your body turns typically under 10 percent of plant ALA into EPA, and under 1 percent into DHA. Eating walnuts is good for you. It is not a substitute for DHA.
How do you read a fish oil label without being misled?
Ignore the number on the front of the bottle. A 1,000 mg capsule refers to the total weight of the oil, not the omega 3 in it, and the combined EPA plus DHA content is often 300 mg or less. The figure that matters is on the back, in the nutrition panel, listed per capsule rather than per serving.
- Find combined EPA plus DHA per capsule. Then check how many capsules make up the stated serving. Two capsule servings are a common way to make a modest product look generous.
- Check the form. Triglyceride and re esterified triglyceride forms are absorbed somewhat better than the cheaper ethyl ester form, though the practical difference is modest.
- Look for a freshness or peroxide value. Oxidised fish oil tastes of fish and offers nothing. If capsules smell strongly on opening, that is a bad sign.
- Vegan? Look for algae, not flaxseed. Flaxseed oil supplies ALA, and ALA is not the thing you are trying to buy.
- Cod liver oil is a different product. It carries vitamin A, which should not be taken in high doses during pregnancy.
Is more omega 3 always better?
No. Above roughly 3 g of combined EPA and DHA a day, benefit plateaus while risks rise. Large cardiovascular trials of high dose omega 3 have reported a higher incidence of atrial fibrillation in the treatment groups, which is a real and specific safety finding rather than a theoretical worry.
Check with a GP or pharmacist before starting a supplement if you take anticoagulants or antiplatelet medicines, if you have a procedure or operation booked, or if you have a fish or shellfish allergy. High doses have a mild blood thinning effect and can interact.
Does omega 3 do anything for hair and skin?
Dietary fatty acids contribute to the skin's barrier, and the scalp is skin, so a diet with adequate fats is part of general scalp condition. What is not true, and gets claimed often, is that eating omega 3 will regrow hair. If you are losing hair, the causes worth investigating are hormonal, nutritional such as low ferritin, thyroid related or stress related, and a GP or trichologist is the right route.
Topical oils work by an entirely different mechanism. They sit on the hair shaft and reduce moisture loss and friction, which is a cosmetic effect on the hair you already have, not a nutritional one.

How long does it take for omega 3 to affect the brain?
Blood levels of EPA and DHA rise within weeks of a consistent change in intake, but red blood cell membrane content, which is the measure used in research, takes roughly 3 to 4 months to stabilise. Any study or product promising a noticeable cognitive change in days is not describing something measurable.
Is fish oil or algae oil better?
Both deliver DHA, and algae oil is where fish get theirs. Algae oil is the better choice for vegans, for anyone who dislikes fish burps, and for people concerned about ocean contaminants. Fish oil is usually cheaper per milligram of combined EPA and DHA, and often supplies more EPA.
Can omega 3 help with mood or depression?
The evidence is more promising for mood than for memory, particularly for formulas higher in EPA than DHA, but the effect sizes are modest and the trials are mixed. Omega 3 is not a treatment for depression. If your mood is low, speak to a GP rather than self treating with supplements.
Do children need omega 3 for concentration?
Trials of omega 3 supplements for concentration and school performance in typically developing children have been largely disappointing. Oily fish is still worth including in a child's diet for general nutrition. Discuss supplements with a GP or health visitor rather than starting them on the basis of advertising.
Does cooking destroy omega 3 in fish?
Gentle cooking such as baking, steaming or grilling preserves most of it. Deep frying is the method that causes the greatest losses, partly through oxidation and partly because the fish absorbs the frying oil. Tinned oily fish retains its omega 3 well, which makes sardines a genuinely convenient option.
Should I take omega 3 if I already eat oily fish twice a week?
Probably not. At that intake you are already meeting the recommendation, and supplements have not been shown to add cognitive benefit on top of an adequate diet. The money is better spent keeping the fish in the shopping basket consistently.
Can you have too much omega 3 from food?
It is difficult from food alone at normal intakes. The UK limits on oily fish are driven by pollutants rather than by the fats themselves: up to 4 portions a week for most adults, and up to 2 during pregnancy or when trying to conceive. Excess is far more likely from high dose capsules.
Does omega 3 prevent dementia?
No trial has shown that it does. Systematic reviews of omega 3 supplements for preventing cognitive decline and dementia in older people have found no clear benefit.
