Hormone therapy can change the thickness of the hairs you grow from now on. It cannot change a hair that has already grown. Every strand on your head is made of keratinised cells that are no longer alive, and its diameter was decided by the follicle at the moment it was produced. So when people say hormone treatment made their hair thicker, what actually happened is that follicles started producing wider fibres at their next handover, and those new fibres took months to become long enough to notice. What does thick hair actually mean?
The word gets used for two completely different things, and the confusion costs people a lot of money.
Fibre thickness is the diameter of a single strand, measured in microns. Hair volume is what you see in the mirror, and it comes from fibre thickness multiplied by how many hairs you have per square centimetre. Someone with fine fibres and very high density can have far more visible hair than someone with coarse fibres and low density.

How thick is a single hair, and what are the fibre types?
Human scalp hair typically measures somewhere in the region of 50 to 100 microns in diameter, with wide variation between individuals and between hair types. For scale, that is roughly the thickness of a sheet of printer paper.

The single most useful idea in hair science sits in that table. Pattern hair loss is largely not follicles disappearing, at least not at first. It is terminal follicles being progressively converted towards vellus, producing a finer and shorter hair each cycle until the hair is too fine and too short to contribute anything visible. Why does the same hormone thicken beard hair and thin scalp hair?
This is the question that makes the whole subject click, and it has a genuinely interesting answer.
Androgens do not carry an instruction that says thicken or thin. They deliver a signal, and each follicle interprets that signal according to where it sits on the body. Beard, chest and body follicles typically respond to androgen exposure by converting vellus fibres towards terminal, which is why facial hair coarsens after puberty and continues to coarsen with androgen exposure. Follicles at the temples and crown of a genetically susceptible scalp respond to the same signal in the opposite direction, converting terminal fibres towards vellus.
The follicles at the back and sides of the scalp are typically far less responsive, which is exactly why that region is used as the donor area in transplant surgery. Hair taken from there tends to keep its original behaviour when moved.
So a single person can be growing a thicker beard and a thinner crown at the same time, from the same hormonal environment, and nothing is malfunctioning. Site sensitivity, largely genetic, is doing the deciding.
How does hormone therapy change fibre thickness?
By changing the signal that follicles receive at their next handover. That is the only lever available, and it works in whichever direction the site sensitivity dictates.
- Reducing androgen activity takes pressure off susceptible scalp follicles. In practice that most often slows further miniaturisation. In some people, follicles that have recently begun to miniaturise return to producing a wider fibre, so the hairs in that area become measurably thicker again. Follicles that have already completed the conversion, or been lost, do not.
- Increasing androgen activity tends to coarsen beard and body hair over months to years, and can accelerate scalp miniaturisation in people who carry the genetic susceptibility.

Important: Do not self prescribe hormones or hormonally active supplements for hair thickness. That includes progesterone creams, compounded or bioidentical hormone products bought without a prescription, and botanical supplements marketed as blocking or balancing hormones. How long does a thickness change take to become visible?
The arithmetic is unforgiving and it explains almost every disappointed review of every hair product ever sold. Hair grows at roughly 1cm a month. A follicle that starts producing a wider fibre today produces a wider fibre only from today onwards, and the old, finer hair above it is still there.

A practical consequence worth knowing: if you cut your hair shorter, you remove the older, finer length and leave a higher proportion of newer growth. The same head of hair can look noticeably better for that reason alone.
Can anything make an existing strand thicker?
Not in true diameter, no. The strand above your scalp is keratinised and no longer living tissue, so nothing can instruct it to grow wider. Anyone who claims otherwise is describing something else.
What products can genuinely do is change how a fibre behaves, which is not nothing:
- Smooth a damaged cuticle. A roughened cuticle catches on neighbouring hairs, tangles and snaps. Smoothing it reduces breakage, so hair keeps the length it grows.
- Deposit a temporary coating. Conditioners and styling products leave a thin film that adds a small amount of measurable width and, more importantly, changes how the hair feels and how light reflects off it. It washes out. That is honest cosmetic function.
- Add hydrolysed proteins. Small protein fragments can adsorb onto and partly fill damaged areas of the fibre surface, temporarily improving strength and reducing breakage. They do not rebuild a hair or change what the follicle produces next.
- Reduce static and separation. Fine hair looks thinner partly because individual strands separate and let scalp show. Controlling that changes apparent volume immediately.
All of that is worth doing, and all of it is cosmetic. It works on the hair you have rather than on the follicle, and it should be described that way.

How do you make fine hair behave like thick hair?
These work today, cost little, and none of them require waiting a year.
- Go shorter. Fine long hair is heavy at the ends and pulls flat. Removing length removes weight, and the roots lift.
- Blunt ends, not heavy layering. Over thinned or heavily razored ends make fine hair taper to almost nothing. A blunter line keeps visible bulk at the perimeter.
- Dry against the fall. Rough drying the roots in the opposite direction to how the hair naturally falls creates lift that lasts the day.

Fibres becoming finer over years, in a pattern, is usually androgenetic alopecia and is worth discussing early because early management protects more than late management does. Book an appointment sooner if you notice:
- A rapid change in fibre quality over weeks or a few months rather than years.
- Hair becoming finer alongside fatigue, weight change, feeling cold, or irregular periods, which can point to thyroid or other systemic causes.
- Fibre change with any scalp symptom: itching, burning, tenderness, scaling or redness.
- Areas of scalp that look shiny and smooth with no visible follicle openings.
- New coarse facial or body hair appearing alongside scalp thinning, which is worth investigating.
Ask about ferritin and iron studies, thyroid function and vitamin D as a starting point, and mention any high dose biotin you take, because it can interfere with several laboratory immunoassays and distort the results.
