This is one of the few claims in the garlic folklore that survives contact with the evidence, which is worth saying plainly given how much of the rest does not. Several meta-analyses pooling randomised controlled trials of garlic preparations against placebo have found a consistent, statistically significant reduction in blood pressure. Three qualifications matter more than the headline number.

Almost every article about garlic tells you to crush the cloves and let them sit for ten minutes, because crushing lets the enzyme alliinase convert alliin into allicin, the sharp smelling compound credited with garlic's effects. The awkward part is that a large share of the blood pressure trials did not use allicin at all. They used aged garlic extract, in which sliced garlic is steeped in dilute ethanol for months. That process destroys allicin almost entirely and leaves behind water soluble sulphur compounds, principally S-allyl cysteine, which is stable, well absorbed and largely odourless. The proposed mechanism is not clot related at all: these compounds appear to support hydrogen sulphide and nitric oxide signalling in the endothelium, which relaxes the smooth muscle in artery walls.
So the preparation with the most blood pressure evidence behind it is the one that smells least like garlic and contains none of the compound everyone credits. That is an inconvenient fact for both camps, and it is the single clearest reason not to assume that heavy raw garlic in your cooking reproduces trial results.

Garlic's blood thinning reputation is a warning label, not a feature
Garlic compounds, particularly ajoene and allicin, do inhibit platelet aggregation in laboratory conditions and to some degree in people. Popular articles present this as garlic being a natural alternative to aspirin. Clinicians read exactly the same finding and reach the opposite conclusion, because an unmeasured, unstandardised, self prescribed antiplatelet effect on top of a prescribed one is not a benefit. It is an uncontrolled variable in a system where bleeding is the thing everyone is trying to avoid.
The practical points that follow:
- Warfarin. Case reports describe raised INR and bleeding when garlic supplements were added. If you take warfarin, do not start a garlic supplement without telling the anticoagulation clinic, and expect closer INR monitoring if you do.
- DOACs, aspirin and clopidogrel. The interaction is less well characterised but the direction of effect is the same. Discuss it rather than assume it is trivial.
- Surgery and dentistry. Most guidance is to stop garlic supplements seven to ten days beforehand, and to declare them at the pre operative assessment. Supplements are the thing patients most often forget to mention because they do not feel like medicines.
- Saquinavir. Garlic supplements substantially reduce blood levels of this HIV protease inhibitor. This is a genuine, clinically meaningful interaction rather than a theoretical one.
- Diabetes medication. Garlic may lower blood glucose slightly, which is worth knowing if you are already treated for diabetes.
- Reflux and stomach upset. High dose garlic on an empty stomach is a common cause of heartburn and nausea, and it is the usual reason people quietly stop taking it.
None of this makes garlic dangerous as a food. It makes concentrated garlic supplements a medicine shaped product that deserves to be treated like one.
What about heart attacks and strokes themselves?
Here the honest answer is short. There is no randomised trial showing that garlic reduces heart attacks, strokes or cardiovascular deaths. How to actually know your blood pressure
This section is probably worth more to you than the rest of the article, because a great many people worrying about garlic have never measured their pressure properly.
In the UK, high blood pressure is generally diagnosed at 140/90 mmHg or above on clinic readings, confirmed by home or ambulatory monitoring at 135/85 or above. The thresholds differ because clinic readings run higher, an effect known as white coat hypertension, which is common and not imaginary. Some people show the reverse pattern, with normal clinic readings and raised readings at home, which is why home measurement matters.
To measure properly at home:
- Use an upper arm monitor with a validated cuff in the correct size. A cuff that is too small overestimates.
- Sit quietly for five minutes first, back supported, feet flat, legs uncrossed, no caffeine or exercise in the previous half hour, and empty your bladder.
- Rest your arm on a table so the cuff sits at heart level, and do not talk during the reading.
- Take two readings a minute apart and record both, morning and evening, for seven days. Discard the first day entirely, then average the rest.
- Bring the whole record to your GP rather than the single worst number, which is what everybody remembers and nobody should act on.

This comes up often enough to deserve a straight answer, because it is a common reason people go looking for natural alternatives in the first place. Some antihypertensives, beta blockers such as propranolol, atenolol and bisoprolol in particular, and occasionally ACE inhibitors, have been associated with telogen effluvium. That is a diffuse shedding across the whole scalp, typically beginning two to four months after starting the drug, and it is usually reversible.
Two things to hold together. It is genuinely uncommon, and shedding that starts around the same time may have nothing to do with the tablet, since illness, surgery, weight loss, low ferritin and thyroid problems all cause the same picture.
