Does thinning hair really run in families?
Thinning hair does run in families when the cause is pattern hair loss, the gradual thinning doctors call androgenetic alopecia, and the NHS says this type usually runs in the family, while shedding after illness or stress and breakage from styling are not passed down in the same way at all.
The strongest numbers come from twins. Rexbye and colleagues photographed the scalps of 739 elderly male twins in Denmark, including 148 complete pairs, and estimated the heritability of balding at 79%. Identical twins matched each other more closely than non-identical twins, and the remaining variation was put down to environmental factors the twins did not share. MedlinePlus Genetics, run by the US National Library of Medicine, adds that the condition tends to cluster in families and that having a close relative with patterned hair loss appears to be a risk factor.
Heritability needs careful reading, though. A figure of 79% describes how much of the difference between people in that group was linked to genes. It is not a 79% chance that you will thin because your father did, and it comes from men over 70, so it says little about women or younger men.
Knowing that genes matter is also easier than knowing which ones. The largest meta-analysis of early male balding at the time, published in Nature Communications in 2017, compared 10,846 men with early-onset hair loss against 11,672 without and found 63 genetic regions that together explained about 39% of the variation. Is it true that baldness comes from your mother's side?
Baldness does not come only from your mother's side: one important gene, the androgen receptor gene, sits on the X chromosome that men inherit from their mothers, but dermatology sources agree that many other genes are involved and that the tendency can be passed down by either parent, or by both.
The myth has a genuine origin. In 2005, Hillmer and colleagues reported in the American Journal of Human Genetics that variation in the androgen receptor gene was the main genetic factor behind early-onset male balding in their study. Because the gene is on the X chromosome, they wrote that its location stresses the importance of the maternal line. So glancing at your mother's father is not a silly idea.
It is just not the whole picture. The British Association of Dermatologists says over 190 genes have been identified as contributing to male pattern hair loss, and that the tendency is believed to be inherited from either or both parents. DermNet states that the susceptibility genes are inherited from both mother and father. A 2017 study of more than 52,000 men aged 40 to 69 in UK Biobank found over 250 independent genetic regions linked to severe hair loss.
- Look at both sides. Your father, both grandfathers and uncles on either side all count.
- Include the women. A mother's, grandmother's or aunt's widening parting is part of the same family picture.
- Note the age it started. Relatives who thinned in their twenties tell you more than a grandparent who thinned at 75.
Why is a parent's hair pattern a clue rather than a verdict?

DermNet explains that several genes are involved, which accounts for differences in age of onset, progression, pattern and severity between family members. The BAD makes the same point, and adds that an earlier start may lead to quicker progression, which is the one piece of family timing worth noting.
Genetic tests do not close the gap yet. DermNet says genetic testing to predict balding is unreliable, and its female pattern page says such testing is currently not recommended. The UK Biobank team built a prediction score from common genetic variants, and of the men it predicted would have severe hair loss, only 59% actually did. That is interesting research, not a forecast to plan your life around, so read any home DNA kit's baldness result with caution.

The useful move is to let family history start you watching early. DermNet describes female pattern hair loss as having a strong genetic predisposition, passed down through many genes from either parent. A 2017 review by Redler, Messenger and Betz in Experimental Dermatology concluded that the role of inherited factors in female pattern hair loss is largely unknown, that no susceptibility gene has been clearly identified, and that its causes may differ substantially from the male form. Both can be true: female thinning often clusters in families, but the genes behind it have not been pinned down, so a mother's thin hair is a weaker guide for a daughter than a father's crown is for a son.
How can you tell pattern thinning from shedding and breakage?
Pattern thinning is a slow narrowing of hairs in a set area over years, shedding is a sudden rise in whole hairs falling out all over the scalp a few months after a trigger, and breakage is the hair shaft snapping part way along, leaving short pieces with no root bulb.
- Hair loss (pattern thinning). Follicles in a set area gradually produce finer, shorter hairs, so the parting widens or the crown shows through. This is the kind that runs in families.
- Hair shedding. The BAD says around 15% of scalp hair is normally in the shedding phase, rising to 30% or more in telogen effluvium, often about 3 months after a trigger such as illness or stress. It does not run in families, and for 1 in 3 people no cause is found.
- Hair breakage. The fibre snaps from heat, bleach, tight styles or rough brushing, and the follicle carries on as normal.
Even the normal daily count differs between sources. The NHS says we can lose between 50 and 100 hairs a day, while the BAD's leaflet says most people lose between 30 and 150. Your own usual amount is the better baseline.

Plenty runs in families without being hair loss: natural hair shape and texture, hair colour and the age you start to go grey are all shaped partly by genes, so fine hair like your mother's or early grey like your father's is not, by itself, a sign that you are losing hair.
A 2016 genome-wide study of more than 6,000 Latin Americans, led by researchers at University College London, found 18 genetic signals linked to hair features, including new regions for scalp hair shape and for balding and the first reported regions for greying. Greying and balding showed up as separate signals, which matches what many families see: grey early on one side, thinning on the other, or both, or neither.
- Fine hair. Each strand is narrow but the number of hairs is normal. Fine hair looks flat, not sparse, and the scalp does not show more over time.
- Early grey. A change of pigment, not of hair count. Grey hair can feel drier and coarser, which affects how it looks, not how much there is.
- A high or uneven hairline. Some people have always had one. Compare against old photographs of yourself, not against a sibling.
What can you do cosmetically if thinning runs in your family?
Cosmetically, the most useful things when thinning runs in your family are a gentle routine that keeps breakage low, a cut and styling that add volume where hair is finer, and a concealer such as hair fibres for a visible parting or crown, plus sun protection for any scalp that now shows.
The BAD's information leaflets mention spray preparations containing small, pigmented fibres as a way to disguise thinning, and advise covering exposed scalp with sun block or a hat, because hair normally shields the scalp from sunlight. When should you speak to your own GP about thinning hair?
Speak to your own GP if hair changes suddenly, if you find smooth bald patches, a sore, red or scaly scalp, or handfuls of whole hairs, or if thinning comes with other symptoms such as tiredness, weight change or irregular periods, because those signs point to causes that family history cannot explain.
The NHS advises seeing a GP to get an idea of what is causing hair loss before thinking about going to a commercial hair clinic, and lists illness, stress, weight loss and iron deficiency among the causes of temporary hair loss. The BAD recommends seeing a doctor if shedding does not get better, to rule out other causes, and classes telogen effluvium lasting more than 6 months as chronic.
